摘要:
目的:分析急性心肌梗死(AMI)合并室间隔穿孔(VSR)患者的临床特点及预后。方法:收集 2008年11月至2019年11月入院的AMI合并VSR患者的临床资料,根据30 d生存状况分为存活组(n=15)和死亡组(n=29),分析该病临床特点和预后影响因素。结果:AMI 合并VSR患者44例,其中女性29例,男性15例,平均年龄(70.5±1.3)岁,前壁AMI 37例(84.1%),Killip心功能分级Ⅲ～Ⅳ级30例(68.2%)。AMI后7 d内出现VSR 37例(84.1%)。17例(38.6%)接受经皮冠状动脉介入术(PCI),12例存活; 16例(36.3%)接受介入封堵治疗,10例存活; 3例(6.8%)接受外科修补术,均存活,VSR从发生到手术的时间平均为(17.8±8.8)d。保守治疗患者30 d死亡率为92.0%,手术(包括介入封堵和外科修补)患者30 d死亡率为31.6%。与存活组患者相比,死亡组患者左室射血分数(LVEF)、合并室壁瘤比例均较低(P<0.05)。Logistic 多因素回归分析表明,年龄(OR=4.956,95%CI:1.031～23.825,P=0.046); PCI(OR=0.049,95%CI:0.005～0.492,P=0.01)、手术治疗(OR=0.056,95%CI: 0.006～0.561,P=0.014)与预后相关。结论:AMI合并VSR多见于女性,前壁AMI,多数患者在AMI后7 d内出现VSR。LVEF较低、无室壁瘤患者的30 d死亡率较高。年龄是30 d死亡率的独立危险因素,而PCI、介入封堵、外科修补术是30 d死亡率的独立保护因素。
