摘要:
慢性心力衰竭是各种心血管疾病的终末阶段,发病率随年龄增长而增加,造成严重的公共卫生负担,开展有效的慢性心力衰竭疾病管理可以减少患者住院率,降低病死率,改善生活质量,减轻患者和社会经济负担。目前常用的慢性心力衰竭疾病管理模式包括以专科医师为主导的医院管理模式、以全科医师为主导的社区管理模式、以护士为主导的电话干预模式和以家庭照顾者为主导的管理模式。
